Prior to approval of any of Globe Telecom's Service Application, a credit check is performed based on the following information: Name of Subscriber, Birthdate and Mother's Full Maiden Name. Upon completion of the Application Form, a credit check follows which determines your eligibility for service application approval. Un-settled payments for services even decades ago will disqualify the applicant immediately. If you are a victim of an identity theft, visit any Globe Wireless Center and request for a copy of the service applied under your name and follow whatever is advised to clear your name.
Above are sample filled-up application form and Identification of Subscriber. Be careful however when filling up the entries, the form has tiny prints so mistakes can be done so easily. My experience however, mistakes are commonly made in the Mother's Maiden Name Section. Some do not even know their mother's maiden name and some don't write the information in the corresponding boxes due to the tiny prints.
